Your First Visit
Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. New patients would receive a comprehensive new patient examination and most commonly take a full mouth series of x-rays at the initial visit, or those x-rays that the doctor feels necessary for proper diagnosis. During your New Patient visit, you and the dentist, hygienist, and team will discuss any findings, recommendations, or desires identified during the visit and create a care plan.
New Patient Forms
In order to keep things simple and keep everything ready prior to your visit, we request that you complete online forms ahead of your visit. Few days prior to your scheduled visit, the office will email / text a link to the forms that you can fill online.
If you are unable to do so, that is okay but we ask that you arrive 15 minutes prior to your new patient appointment so we can do the paperwork prior to your appointment.
